About the position

The Behavioral Health Refugee Services Case Manager is a Master's level role focused on providing comprehensive case management services to refugees and vulnerable populations. This position involves social casework, engagement, mentoring, and community referrals, extending beyond the initial resettlement period. The Case Manager will conduct mental health screenings and make appropriate referrals to mental health care providers, ensuring clients receive the necessary support in medical, educational, housing, and special needs areas.

Responsibilities

  • Assumes primary responsibility for all persons served on assigned caseload, including intake, assessment, periodic reviews, and treatment planning.
  • Responsible for all reports, progress notes, collateral contacts and assignments as required.
  • Provide a range of case management, including social casework, engagement, mentoring, and community referrals to service providers.
  • Conduct a mental health screening for all clients and make timely, appropriate referrals to mental health care providers.
  • Provide information, referrals, and outreach services regarding community resources, including creating and distributing a resource guide.
  • Participate with Clinical Manager and other staff in care planning and goal setting for the growth and development of vulnerable individuals served.
  • Coordinate the development, implementation, and evaluation of Person-Centered Planning.
  • Work collaboratively with referral sources, agencies, institutions, and individuals who have important roles in the client's life situation.
  • Participate in in-service trainings and conferences related to the program.

Requirements

  • Master's degree in a human service-related field, or equivalent experience working with refugees or other vulnerable populations.
  • Minimum one year experience in case management in a culturally diverse environment, refugee related casework or community services preferred.
  • Ability to work effectively in a cross-cultural context.
  • Excellent organization and prioritization skills; ability to utilize computer technology and various applications, including spreadsheets.
  • Ability to professionally and effectively communicate orally and in writing.

Nice-to-haves

  • Understanding or willingness to quickly learn the characteristics, behaviors and needs of the refugee populations.
  • Ability to facilitate and collaboratively work with the treatment team to establish and implement treatment plans and goals.
  • Ability to develop outcomes measurements related to Person Centered Planning goals.
