About the position

The Regional RN Care Manager is a key leadership role within a growing for-profit healthcare organization, responsible for overseeing clinical services and reimbursement processes for Skilled Nursing Homes across multiple states. This position involves managing a team of Care Managers to ensure quality clinical outcomes and effective resource utilization, while also ensuring compliance with Medicaid, Medicare, and other payor regulations.

Responsibilities

  • Oversee Medicare, Medicaid, and other payor sources to ensure appropriate clinical services are provided.
  • Manage a team of 4 - 5 Care Managers responsible for care plan management.
  • Focus on quality clinical outcomes and maximum resource utilization.
  • Assess patients' status and progress to create and modify Care Plans.
  • Ensure compliance with Healthcare DOH regulations and state policies.
  • Meet department and company goals.

Requirements

  • Must be a licensed RN.
  • A BSN degree is preferred.
  • Minimum of 3-4 years of experience in Skilled Nursing Homes.
  • Experience working with multiple facilities in multiple states.
  • Minimum of 2 years in a supervisory or management role.
  • Deep understanding of Medicaid, Medicare, and Managed care reimbursements, regulations, and policies.
  • Proficient computer skills and EMR software.
  • Excellent managerial, interpersonal, and communication skills.
