About the position

The Regional Navigator Case Manager is a key member of the care team responsible for establishing and maintaining effective relationships with youth, families, referral sources, and community resources throughout the care process. This role supports the Regional Navigator Program for youth and young adult survivors of human trafficking, acting as a single point of contact to ensure organized service provision and effective communication. The position involves conducting comprehensive needs assessments, ongoing safety planning, and promoting resources to enhance self-sufficiency for youth and families. Family engagement and collaboration with community partners are essential aspects of this role, aimed at advocating for youth needs and increasing public awareness through education and advocacy.

Responsibilities

  • Initiate and maintain effective working relationships with youth, families, referral sources, and community resources.
  • Support the Regional Navigator Program for youth and young adult survivors of human trafficking.
  • Act as a single point of contact for effective communication and organized service provision.
  • Develop and review comprehensive needs assessments and ongoing safety planning with youth.
  • Promote and identify community resources to enhance self-sufficiency for youth and families.
  • Engage families and identify their strengths and needs.
  • Collaborate with community partners for consistent screening and referrals of suspected survivors of child sex trafficking.
  • Advocate for youth needs and conduct warm handoffs to appropriate services.
  • Participate in care coordination meetings and court appointments as needed.
  • Complete ongoing functional assessments with participation from youth and families.
  • Maintain effective working relationships with families, colleagues, and community agencies.
  • Ensure post-discharge follow-up to assess the impact of treatment.
  • Provide effective case coordination and referrals to internal and external professionals.
  • Complete required documentation and reports within defined timelines.

Requirements

  • Bachelor's Degree.
  • At least one year of experience in providing direct services to children/youth and families.
  • Experience working with survivors of trafficking, sexual assault, domestic violence, or other trauma.
  • Experience in child and young adult systems of care or multi-disciplinary teams.
  • Experience in emergency or crisis response services.

Nice-to-haves

  • Experience in community outreach and education regarding human trafficking.
  • Familiarity with trauma-informed care practices.
