About the position

The Regional Physician Lead at TapestryHealth is responsible for conducting Initiating Patient Visits (IPVs) to nursing home residents, confirming chronic medical conditions, providing education on services, and obtaining consent for enrollment in Chronic Care Management (CCM) and Remote Physiological Monitoring (RPM) services. This role involves oversight of a multistate clinical team, ensuring compliance, driving clinical quality assurance, and collaborating across various functions to enhance patient care and outcomes.

Responsibilities

• Conduct approximately 300 clinical visits onsite or via telehealth per month.
• Document patient encounters accurately and thoroughly within our platform.
• Provide clinical Collaborative Practice Agreements for Advanced Practice Providers (APPs) in the region.
• Review patient charts, medical records, and treatment plans for accuracy and compliance.
• Drive quality assurance for Chronic Care Management and Remote Physiological Monitoring with monthly sampling of reports.
• Provide guidance and feedback to APP team to enhance clinical skills and improve patient outcomes.
• Serve as final reviewer and billing practitioner for regional CCM reports.
• Track key performance indicators and measure patient outcomes in partnership with the regional team.
• Act as lead clinical liaison to facility-based medical teams for effective adoption of CCM service programs.
• Partner with business development team to educate corporate clinical leadership during new client launches.

Requirements

• Qualified MD/DO with active, unrestricted medical licenses in the assigned region.
• Excellent clinical judgment and decision-making skills.
• Ability to work independently and remotely as part of a multidisciplinary team.
• Commitment to providing patient-centered care and improving health outcomes.
• Familiarity with electronic health record (EHR) systems and healthcare technology.
• Proficient with Microsoft Office Suite.
• Ability to travel within the designated region when needed.
