About the position

The RN Care Manager plays a crucial role in ensuring patient-centered care coordination and progression through the continuum of care. This position involves collaboration with patients, families, social workers, nurses, physicians, and the interdisciplinary team to facilitate efficient and cost-effective care. The RN Care Manager is responsible for evaluating post-hospital needs, developing transition of care plans, and ensuring smooth patient flow and satisfaction while adhering to regulatory compliance and quality patient care standards.

Responsibilities

• Ensure patient-centered care coordination and progression through the continuum of care.
• Conduct initial evaluations for transition of care needs within one calendar day of admission.
• Collaborate with the interdisciplinary team during daily multidisciplinary rounds.
• Develop and implement transition of care plans prior to patient discharge.
• Educate nurses, physicians, and the interdisciplinary team on resource utilization and medical necessity.
• Monitor patient readmission risk scores and coordinate mitigation interventions.
• Facilitate patient care conferences with the multidisciplinary team as needed.
• Assist with end-of-life conversations and advance directives.
• Document patient notifications for compliance with Medicare regulations.

Requirements

• Associates Degree in Nursing
• Current valid State of Florida or multistate license as a Registered Nurse
• 2 years of medical/hospital nursing experience

Nice-to-haves

• Bachelor's degree in Nursing
• Health-related master's degree or MSN
• Prior Care Management/Utilization Management experience
• Professional Certification
