About the position

The RN Care Manager plays a crucial role in ensuring patient-centered care coordination and progression through the continuum of care. This position involves collaboration with patients, families, and an interdisciplinary team to facilitate efficient and cost-effective care, focusing on optimal patient flow, safety, satisfaction, and readmission prevention. The RN Care Manager is responsible for evaluating post-hospital needs, developing transition of care plans, and ensuring quality patient care and regulatory compliance.

Responsibilities

  • Collaborate with patients, families, and interdisciplinary teams to ensure patient-centered care coordination.
  • Conduct initial evaluations for transition of care needs within one calendar day of admission.
  • Review necessary patient information including labs, medications, and medical history for care planning.
  • Incorporate patient/family care goals and preferences into transition of care planning.
  • Meet with patients and families to discuss discharge options and post-hospital care providers.
  • Identify and resolve barriers to transition of care plan achievement with the interdisciplinary team.
  • Evaluate potential for readmissions and coordinate mitigation interventions.
  • Consult with Social Work for specialty services related to psychosocial needs.
  • Develop and document discharge plans with contingency plans throughout the hospital stay.
  • Facilitate patient care conferences with the multidisciplinary team as needed.
  • Participate in daily multidisciplinary rounds to review care progression and discharge plans.
  • Proactively identify patients who no longer meet medical necessity and escalate potential denials.
  • Ensure timely patient notifications for compliance with regulations.

Requirements

  • Associates Degree in Nursing
  • Current valid State of Florida or multistate license as a Registered Nurse
  • 2 years of medical/hospital nursing experience

Nice-to-haves

  • Bachelor's degree in Nursing
  • Health-related master's degree or MSN
  • Prior Care Management/Utilization Management experience
  • Professional Certification
