About the position

The Registered Nurse Care Manager/Coordinator at the VA Texas Valley Coastal Bend Health Care System is responsible for managing complex patient care through leadership and innovative approaches. This role involves collaborating with a team to develop patient-driven care plans, providing health education, and ensuring quality outcomes for patients. The nurse will utilize advanced clinical knowledge to assess, plan, implement, and evaluate care across the continuum, while also serving as a clinical resource and educator for the team.

Responsibilities

  • Collaborate with the Teamlet and larger Team to develop patient-driven holistic care plans.
  • Provide patient and family health education focused on self-management, prevention, and wellness.
  • Follow personalized health care plans designed by the patient and Patient Aligned Care Team (PACT).
  • Demonstrate advanced clinical knowledge in assessing, planning, implementing, documenting, and evaluating care for a designated group of patients.
  • Triage and apply a collaborative team approach in identifying, analyzing, and resolving patient care problems.
  • Utilize time management and communication skills to facilitate the clinic's day-to-day mission.
  • Ensure timely triage of patients within 45 minutes of check-in and complete appointments within 30 minutes.
  • Provide indirect patient care in collaboration with the interdisciplinary team and serve as a clinical resource expert.
  • Monitor patient progress and intervene as necessary to achieve desired outcomes within anticipated timeframes.
  • Review and address pending alerts communicated throughout the tour of duty to facilitate care within 48 hours.

Requirements

  • U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens.
  • Graduation from a school of professional nursing approved by the appropriate accrediting agency.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a state, territory, or commonwealth of the U.S.
  • Proficiency in basic written and spoken English.

Nice-to-haves

  • Recent ambulatory care experience (1-3 years).
  • Med/Surgery/ED experience (1-3 years).
  • Recent care coordination experience (1-3 years).
  • Outpatient care experience.
  • Telehealth nursing experience.
