About the position

The position involves providing home health care services to patients in the Hillsborough County area. The role focuses on performing initial assessments, developing care plans, implementing medical treatments, and coordinating services with other healthcare providers. Responsibilities

• Perform the initial assessment of the patient and caregiver abilities for the appropriateness of and acceptance for home health services.
• Establish and periodically review the plan of care.
• Implement medical treatment when ordered.
• Follow up on referrals and provide, evaluate, and supervise care.
• Coordinate services given by other health care providers and document all activities and findings.
• Complete documentation according to Organizational Policies and Regulatory guidelines.
• Perform home visits as ordered.

Requirements

• Graduate of an approved school of professional nursing.
• Minimum of 1 year of clinical experience in Medical/Surgical nursing.
• Current Florida licensure as a Registered Nurse.
• Current BCLS certification.
• Valid Driver's License.

Nice-to-haves

• Two years of Home Health Nursing experience.
• Bachelor's Degree in Nursing.
• IV therapy skills desirable.
• Venipuncture skills desirable.
