About the position

The Registered Nurse Care Manager for Maternity at Intermountain Health is responsible for partnering with members, their families, and healthcare providers to facilitate self-management of care, optimize health outcomes, and ensure appropriate use of the healthcare system. This role involves developing and implementing care plans that address various needs, conducting assessments, and coordinating communication among all parties involved in the care process. The position is primarily remote with occasional in-office meetings and training, reporting to a Healthy Connections Director.

Responsibilities

  • Identify members needing assistance with medical plans and enroll them in care management services.
  • Assess, establish, and maintain individualized care plans based on members' needs and goals.
  • Ensure care plans are evidence-based and align with referral goals and payer contracts.
  • Maintain accurate records and case notes in compliance with regulatory standards.
  • Facilitate communication among members, families, and healthcare teams through interdisciplinary meetings.
  • Monitor patient progress and adjust care plans as necessary, referring to higher levels of care when indicated.

Requirements

  • Current RN compact license to practice in all states that are part of the compact.
  • Demonstrated experience in clinical nursing within an ambulatory care setting.
  • Experience in care management, managed care, health promotion, or patient education roles.
  • Strong written and verbal communication skills, along with conflict resolution abilities.
  • Experience working in a remote environment and proficiency in Microsoft Suite, including Teams and Outlook.

Nice-to-haves

  • Bachelor's degree in Nursing (BSN) from an accredited institution.
  • Case Management Certification.
  • Three years of experience in clinical nursing in ambulatory care, community health, or home care settings.
  • Experience with third-party payers.
  • Ability to work independently and adapt to a rapidly changing environment.
