About the position

Spectrum Healthcare Resources is seeking a Full-time Registered Nurse - Behavioral Health/Case Manager for the Fairfax Ambulatory Care Clinic in Fairfax, VA. This contract position involves working in an outpatient setting, providing care to active duty personnel, retirees, and their families. The role focuses on triaging patients, coordinating care, and collaborating with healthcare teams to manage high-risk patients effectively.

Responsibilities

• Triage patients to determine the nature and urgency of Behavioral Health problems.
• Assist patients in finding appointments in the direct and purchased care BH system.
• Facilitate emergent care coordination and transfers for inpatient psychiatric hospitalization.
• Collaborate and plan for comprehensive health care with other healthcare team members.
• Monitor, track, and manage patients identified as 'high risk' in collaboration with the Behavioral Health provider team.
• Function as a resource person for patients, helping them access available community resources.
• Support ongoing case management efforts for Behavioral Health patients.

Requirements

• Must be a U.S. Citizen.
• Bachelor of Science Degree in Nursing (BSN) from an accredited college or university.
• 2+ years of RN Behavioral Health experience preferred within the last 5 years.
• Working knowledge of interdisciplinary treatment team processes and behavioral health programming.
• Proficiency in computer operations and basic word processing.

Nice-to-haves

• Experience in outpatient behavioral health settings.
• Knowledge of case management tracking and assistance.
