About the position

The Registered Nurse, Case Manager & Care Coordinator Team Lead at AltaMed is responsible for managing the care of PACE participants in an ambulatory setting. This role involves overseeing case management activities under the supervision of the Nursing Supervisor and PACE Physicians, ensuring that quality care is delivered to all participants. The position emphasizes a commitment to community health and the professional growth of its staff.

Responsibilities

• Case manage all PACE participants in the ambulatory setting.
• Oversee care coordination activities and ensure quality care delivery.
• Collaborate with PACE Physicians and nursing staff to develop care plans.
• Monitor and evaluate the effectiveness of care plans and make adjustments as necessary.
• Provide guidance and support to nursing staff in case management practices.

Requirements

• Current valid License as a Registered Nurse through the California Board of Registered Nursing.
• A minimum of one-year experience in Geriatric Nursing required.
• A minimum of one-year experience preferred in managed care setting, preferably in utilization or case management in ambulatory or inpatient setting.
• Current valid BLS/First Aid Certificate required.
• Bilingual English and Spanish preferred.
