About the position

The Registered Nurse Case Manager in Hospice is responsible for delivering comprehensive nursing care to hospice patients in their homes. This per diem position emphasizes building trusting relationships with patients and their families while adhering to the hospice's policies and procedures to ensure quality care. The role involves developing and implementing individualized care plans in collaboration with other healthcare professionals, focusing on the physical, emotional, and spiritual well-being of patients at the end of life.

Responsibilities

• Assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient.
• Continuously evaluate personal and professional performance to enhance productivity and quality of care.
• Adhere to and support the policies, procedures, goals, and objectives of hospice to assure quality patient care.
• Develop the patient's Plan of Care in collaboration with other professionals.
• Implement treatment strategies based on scientific nursing theory that promotes overall well-being.
• Communicate effectively with patients, families, and team members, both verbally and in writing.

Requirements

• Current RN licensure in the state of practice.
• One year of clinical experience in nursing.
• Current CPR Certification.
• Current driver's license and valid vehicle insurance.
• Access to a dependable vehicle or public transportation.
