About The Position
The RN Case Manager at Luminos Hospice is responsible for leading a care team to provide exceptional hospice care. This role involves coordinating patient care, collaborating with healthcare teams, and ensuring that patients receive the best end-of-life experience. The RN Case Manager will work closely with the Clinical Director and other team members to implement care plans and support families during this critical time.
