About the position

The RN Case Manager in Hospice at Baptist Home Care and Hospice is responsible for delivering comprehensive nursing care to hospice patients, ensuring their physical, psychosocial, and environmental needs are met. This role emphasizes building trusting relationships with patients and their families, coordinating care plans, and collaborating with interdisciplinary teams to provide high-quality end-of-life care. The position requires a commitment to continuous evaluation and improvement of care delivery, as well as participation in various administrative and supervisory tasks.

Responsibilities

  • Assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient.
  • Make the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
  • Identify and reassess the patient's/family's needs no less than every 15 days.
  • Initiate and coordinate the plan of care while maintaining accurate clinical notes.
  • Document problems, goals, interventions, and patient/family responses to care.
  • Collaborate with the patient/family, attending physician, and IDG members in daily care.
  • Instruct and supervise the patient/family in self-care techniques when appropriate.
  • Report patient condition and identified needs/issues to supervisor daily.
  • Supervise and evaluate home health aides' performance every 14 days.
  • Perform supervisory aide visits to assess goal achievement every 14 days.
  • Complete required hospice assigned modules online monthly.
  • Participate in daily nursing reports and provide necessary documentation.
  • Perform 60 or 90 day record reviews for recertifications as appropriate.
  • Follow processes for admissions and recertification of hospice patients.
  • Participate in IDG meetings regarding patient care every 14 days.
  • Engage in QAPI plan and process daily, identifying problems to the Administrator.
  • Ensure compliance with medication ordering and administration processes.
  • Maintain an up-to-date personnel file.
  • Perform LPN supervisory visits as assigned by supervisor.
  • Participate in on-call rotation.

Requirements

  • Current RN licensure in the state of practice.
  • One year of clinical experience as an RN.
  • Current CPR Certification.
  • Current driver's license and valid vehicle insurance.
