About the position

The Registered Nurse - Case Manager position is a remote role that involves conducting medical management and health education programs for customers enrolled in government health care programs. The role requires field visits to perform face-to-face health screenings and assessments, either in-home or at facility sites. The case manager will gather, analyze, and provide data for regulatory reports while representing the company to its members.

Responsibilities

• Conduct medical management and health education programs for customers on government health care programs.
• Perform face-to-face health screenings and assessments in assigned territory.
• Gather, analyze, and provide data for regulatory reports.
• Represent the company to members and communicate medical needs effectively.

Requirements

• Registered Nurse (RN) with 3 years of direct clinical care experience in a clinical setting, or Licensed Professional Counselor (LPC), or Licensed Master Social Worker (LMSW) with 2 years of clinical practice.
• Current, valid, unrestricted RN license in the state of operations or reciprocity.
• 3 years of wellness or managed care experience presenting clinical issues with members/physicians.
• Strong verbal and written communication skills for discussing medical needs with members and interfacing with internal staff and external vendors.
• Analytical experience including medical data analysis.
• Current IL driver's license, transportation, and applicable insurance.
