About the position

The Registered Nurse Case Manager (RNCM) position at North Kansas City Hospital Home Health involves providing home-based nursing care to patients, ensuring they receive quality care in their preferred environment. The RNCM will coordinate care, educate patients and their families, and work as part of an interdisciplinary team to enhance patient outcomes. This role emphasizes autonomy, leadership, and the ability to manage patient care effectively while adhering to agency protocols and regulations.

Responsibilities

• Plan and monitor patient conditions and perform OASIS assessments to develop individualized care plans.
• Administer medications as prescribed and prioritize visits for high-risk patients to minimize re-hospitalizations.
• Oversee and supervise the care provided by nurse aides and LPNs.
• Manage multi-disciplinary care to promote continuity during admissions, transfers, and discharges.
• Educate patients and families on disease processes using teach-back methods to ensure understanding.
• Complete clinical documentation following agency protocols and Medicare/Federal guidelines.
• Collaborate with the interdisciplinary team and physicians to ensure optimal patient care.
• Attend in-service training and mandatory agency meetings.

Requirements

• Registered Nurse with a current license in the state of employment.
• Minimum one-year experience as an RN in an acute care setting; home care experience preferred.
• Valid driver's license and auto insurance in your name as a driver.
• Capable of all physical demands.
