About the position

The Registered Nurse Case Manager (RNCM) at North Kansas City Hospital Home Health is responsible for providing home-based nursing care to patients, ensuring they receive quality care in their preferred environment. This role involves coordinating care, developing individualized care plans, and working closely with an interdisciplinary team to enhance patient outcomes. The RNCM will also educate patients and their families, manage clinical care under physician direction, and oversee the work of nurse aides and LPNs.

Responsibilities

• Plan and monitor patient conditions and perform OASIS assessments to develop individualized care plans.
• Administer medication as prescribed and prioritize visits for high-risk patients to minimize re-hospitalizations.
• Oversee and supervise total care provided by nurse aides and LPNs.
• Manage multi-disciplinary care to promote continuity of care during admissions, transfers, and discharges.
• Educate patients and families on disease processes using teach back methods to ensure understanding.
• Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
• Collaborate with the interdisciplinary team and physicians to ensure optimal care is provided.
• Attend in-service training and mandatory agency meetings.

Requirements

• Registered Nurse with current license in the state of employment.
• Minimum one-year experience as an RN in an acute care setting.
• Home care experience preferred.
• Valid driver's license and auto insurance in your name as a driver.
• Capable of all physical demands.
