About the position

As a Registered Nurse (RN) Case Manager at CommonSpirit Health Mountain Region, you will play a crucial role in advocating for patients and ensuring they receive the best possible care throughout their healthcare journey. This position involves collaborating with interdisciplinary healthcare partners, facilities, and families to facilitate care coordination and connect patients with necessary resources. Your expertise will be essential in assessing patient needs, managing care utilization, and ensuring compliance with medical appropriateness criteria.

Responsibilities

• Advocate for patients and collaborate with healthcare partners, facilities, and families.
• Utilize clinical expertise and independent judgment in assessing and reassessing patient needs.
• Facilitate care coordination and utilization management for patients.
• Ensure medical appropriateness criteria are met for patient status and level of care.

Requirements

• Nursing Diploma or Associate's Degree in Nursing required; BSN preferred.
• RN License in applicable state of employment required.
• 1 year of nursing or case management experience required.
• Experience working with EMR preferred.
• Working knowledge of regulatory requirements and accreditation standards strongly preferred.
• BLS from the American Heart Association required.
