About the position

The Hospice Registered Nurse Case Manager (RNCM) at Care360 Hospice is responsible for coordinating hospice care for patients and their families. This role involves working collaboratively with a team to ensure that patients' needs are met, conducting evaluations, and maintaining comprehensive care plans. The RNCM will also supervise hospice aides and participate in case conferences, ensuring high-quality care is delivered to patients in their homes.

Responsibilities

• Coordinate hospice care by collaborating with team members and family members.
• Perform evaluations for hospice care and conduct informational visits as needed.
• Consult with the attending physician and hospice medical director regarding changes in patient conditions.
• Submit complete, accurate, and relevant clinical notes promptly.
• Participate in IDG and case conferences.
• Conduct initial and ongoing comprehensive assessments of patients.
• Develop and implement the initial plan of care and maintain it with the IDG team.
• Supervise and evaluate care provided by hospice aides at least once every 14 days.
• Provide care plans to hospice aides and LPNs as part of the IDG team.
• Travel to patients' residences to conduct visits.

Requirements

• Licensed Registered Nurse (RN)
• CPR/BLS Certified
• Valid Driver's License and Auto Insurance Coverage
• Reliable transportation
• Minimum of 1 year of clinical experience
• Hospice experience preferred
