About the position

The Registered Nurse Case Manager at Innovive Health is responsible for providing high-quality home healthcare services to patients with chronic behavioral health issues and complex medical needs. This role involves working one-on-one with patients in a homecare setting, ensuring the best outcomes through collaboration with a team of skilled caregivers and nurses. Responsibilities

• Provide one-on-one nursing care to patients in their homes.
• Collaborate with a team of caregivers and nurses to ensure optimal patient outcomes.
• Manage patient care plans and coordinate with healthcare providers as needed.
• Conduct assessments and monitor patient progress.
• Educate patients and their families about health management and care plans.

Requirements

• Graduate of an accredited school of professional nursing.
• Current RN license in good standing to practice in Colorado.
• Minimum of 1 year of RN experience.
• Valid CPR certification.
• Must be a licensed driver with an insured automobile or reliable transportation.
• Successful completion of Fit Testing for N95 Respirators.
