About the position

The RN Hospice Case Manager at Phoenix Home Care and Hospice is responsible for providing compassionate nursing care to clients and their families facing terminal diagnoses. This role involves working collaboratively with an interdisciplinary team to develop and implement patient-centered care plans, facilitating quality of life through effective communication and guidance in healthcare decisions.

Responsibilities

• Provide education to patients and family members regarding the process and plan of care.
• Consult with the attending physician and Hospice Medical Director regarding changes in patients' conditions.
• Coordinate hospice care by collaborating with team members and family members to meet patient needs.
• Conduct initial and ongoing comprehensive assessments of patients.
• Perform evaluations for hospice care and conduct informational visits.

Requirements

• Missouri or Compact RN License
• Hospice or Long-Term Nursing experience
• Valid driver's license and auto liability insurance.
