About the position

The Case Manager, Registered Nurse (RN) at Cardinal Hospice is responsible for organizing and directing hospice care services, focusing on community health education and patient needs. This role involves assessing patients and families, developing care plans, and providing health care instructions while ensuring high-quality care and support for residents and their families.

Responsibilities

• Completes initial assessment of patient and family to determine hospice care needs.
• Regularly re-evaluates patient nursing needs.
• Initiates plan of care and makes necessary revisions as patient status and needs change.
• Counsels family and patient in meeting nursing and related needs.
• Provides health care instructions to patient as appropriate per assessment.
• Communicates with physician regarding the patients needs and reports any changes.
• Participates in on call duties as defined by policy.

Requirements

• Clear and active Registered Nurse, RN license in North Carolina
• Graduate of an accredited school of nursing
• 2 years of Hospice experience, preferred
• Clear and valid Drivers License
• Current CPR certification
• Supervisor skills
• Successful criminal background check
• Subject to pre-employment drug testing
• Willingness to travel 75% of the time
