About the position

The RN Case Manager at Elite Hospice is responsible for delivering comprehensive end-of-life care to hospice patients. This role emphasizes building trusting relationships with patients and their families, ensuring they feel valued and respected. The position involves continuous evaluation of care plans and collaboration with interdisciplinary teams to meet the physical, psychosocial, and environmental needs of patients.

Responsibilities

• Assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient.
• Make the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identify and reassess the patient's/family's needs no less than every 15 days.
• Initiate and coordinate the plan of care while maintaining accurate clinical notes.
• Document problems, goals, interventions, and patient/family responses to care.
• Collaborate with patients, families, attending physicians, and other team members in daily care.

Requirements

• Current RN licensure in the state of practice.
• One year of clinical experience.
• Current CPR Certification.
• Current driver's license and valid vehicle insurance, or access to public transportation.
