About the position

The Hospice Registered Nurse provides direct nursing care to patients with terminal illnesses who have chosen the Hospice benefit. This role involves assessing patient needs, developing care plans, educating patients and their families, and evaluating care outcomes. The nurse coordinates care with other healthcare team members and offers supportive services to families and significant others.

Responsibilities

• Provide direct nursing care including assessment, planning, education, and evaluation for hospice patients.
• Coordinate the delivery of care with other care team members and disciplines.
• Support patients' families and significant others during the care process.
• Maintain the flow of patient care duties as necessary.

Requirements

• Current California RN license
• Current Basic Life Support Card
• Valid California Driver's license and automobile insurance
• Minimum one (1) year experience as a professional nurse
• Clinical knowledge to assist patients with terminal care needs
• Basic organizational skills
• Basic computer skills
• Demonstrated leadership abilities
• Patient assessment skills
• Clinical education related to specialty
• Knowledge of Hospice Condition of Participation

Nice-to-haves

• Medicare Certified Hospice experience
• Public Health Nurse certification
• Certified Hospice and Palliative Nurse
• BSN/MSN degree
