About The Position

The Registered Nurse Patient Aligned Care Team (PACT) Outpatient is responsible for providing comprehensive, patient-driven holistic care to Veterans. This role involves collaboration with Primary Care Providers and other team members to manage chronic diseases and ensure effective care delivery. The nurse will oversee direct patient care, focusing on education, self-management, and customer satisfaction, while adhering to established policies and guidelines.

Requirements

• U.S. Citizenship is required; non-citizens may only be appointed when qualified citizens are unavailable.
• Proficiency in basic written and spoken English.
• Graduation from an accredited school of professional nursing.
• Current, full, active, and unrestricted registration as a professional nurse in a U.S. state or territory.

Nice To Haves

• Experience in a healthcare setting with Veterans or similar populations.
• Knowledge of chronic disease management and patient education techniques.
