About the position

The RN Case Manager at Lifespark is responsible for planning, organizing, and directing services while ensuring compliance with regulatory requirements and best practices. This role involves developing care plans, supervising caregivers, and ensuring client satisfaction, all while working within the community to meet the needs of individuals and families in their homes.

Responsibilities

• Plan, organize, and direct Lifespark services.
• Ensure compliance with regulatory requirements and best practices.
• Complete starts of care and develop care plans.
• Provide ongoing support and ensure client satisfaction.
• Supervise and engage caregivers during service delivery.
• Onboard new caregivers and staff.

Requirements

• 2 years' RN experience in health care, preferably in home care or hospice.
• Medicare Skilled Nursing experience and basic understanding of OASIS strongly preferred.
• Experience in customer service or direct member contact field preferred.
• Experience supervising caregivers.
• Knowledge of Medicare Home Health & Hospice regulations.
• Knowledge of corporate business management, governmental regulations, ACHC standards, and private payer practice.
• Graduate of an accredited school of nursing.
• Current Minnesota Nursing License.
• Must pass a MVR check based on Lifespark company policy.
