About the position

The RN Case Manager position at Banner Health is a full-time role focused on providing comprehensive care coordination for patients. The role emphasizes building relationships with patients and their families to ensure effective discharge planning and continuity of care. The RN Case Manager will be an active change agent, dedicated to delivering high-quality clinical care and addressing barriers to effective patient care. This position requires a commitment to safe, evidence-based practices and involves managing individual patients across the healthcare continuum.

Responsibilities

  • Manage individual patients across the healthcare continuum to achieve optimal clinical care, financial, operational, and satisfaction outcomes.
  • Act in a leadership function with process improvement activities for populations of patients.
  • Collaboratively develop and manage the interdisciplinary patient discharge plan, effectively communicating it across the continuum of care.
  • Maintain knowledge of Medicare, Medicaid, and other program benefits to assist patients with discharge planning.
  • Establish and promote collaborative relationships with physicians, payers, and other healthcare team members.
  • Educate internal members of the healthcare team on case management and managed care concepts.
  • May supervise other staff and determine how to best accomplish functions within established procedures.

Requirements

  • Must possess knowledge of case management or utilization review, typically obtained through a bachelor's degree in case management or healthcare.
  • Requires a current Registered Nurse (R.N.) license in the state of work.
  • Basic Life Support (BLS) certification is required for assignments in an acute care setting.
  • Requires 2-3 years of clinical experience and proficiency in critical thinking, problem-solving, effective communication, and time management skills.
  • Ability to work effectively in an interdisciplinary team format.

Nice-to-haves

  • Certification for CCM (Certified Case Manager) preferred.
  • Additional related education and/or experience preferred.
