About the position

The Registered Nurse (RN)/Case Manager position at Harborview Medical Center's GI Clinic involves providing specialized nursing care and case management for patients in a multi-disciplinary outpatient setting. The role emphasizes collaboration with various healthcare professionals to ensure comprehensive patient care, effective communication, and continuity of care across different services. The RN will also engage in professional development activities and may take on charge nurse responsibilities as needed.

Responsibilities

  • Serve as a liaison between the Endoscopy unit, staff from Harborview and outside hospitals, patients and patient families, referring physicians, outside agencies, and the HMC Gastroenterology service.
  • Assume the responsibility of charge nurse as assigned by the AACS clinic practice manager and/or RN3.
  • Participate in clinic and selected divisional activities to promote delivery problem solving and communication and continued professional development for self and others.
  • Demonstrate proper sterile/clean technique for all procedures including blood draws (simple and complex), IM, and SQ injections.
  • Assess patients coming in for nurse triage, symptom management, and phone triage, resolving patient problems where appropriate.
  • Enter outpatient therapy plans and coordinate scheduling of infusions both internal and with outside infusion providers.
  • Communicate needs to PSS's and PCC's when people need to be scheduled for clinic visits/procedures.
  • Assist with prior authorizations of medications, medication teaching, and helping patients access financial assistance programs.
  • Delegate responsibilities within the job/position description and performance role expectations of supporting staff.
  • Facilitate continuity of care across the continuum of care.

Requirements

  • Active, valid Washington State or Multi-state RN license required by start date.
  • Two years of professional nursing experience.
