About the position

The Home Health Registered Nurse (RN) Care Manager is responsible for coordinating and directing the care of home health patients based on their individual needs. This role involves independent management of a caseload, utilizing advanced assessment, teaching, and decision-making skills to ensure effective patient care. The RN Care Manager collaborates with physicians and other healthcare providers to develop and implement personalized care plans, while also educating patients and their families about their care.

Responsibilities

• Coordinate and direct the care of a caseload of home health patients.
• Conduct comprehensive assessments, planning, implementation, and evaluation for the caseload as the primary nurse.
• Set priorities for home care caseload based on changing patient needs.
• Optimize daily schedule to support productivity and maintain best practice visit utilization.
• Assess physical, functional, psychosocial, social, spiritual, educational, developmental, cultural, cognitive status, and discharge planning needs of home care patients.
• Evaluate the home environment for safety and community resource needs.
• Review patient history, diagnostics, and laboratory data, reporting abnormal results to the physician as appropriate.
• Formulate patient-specific plans of care in consultation with patients, families, and physicians.
• Establish individualized, realistic, measurable patient-centered goals.
• Implement the plan of care through direct patient care and coordination of the healthcare team.
• Provide skilled nursing care and preventative rehabilitative procedures.

Requirements

• Current RN License in State of Practice
• CPR certified
• Valid Driver's License and current car insurance
• Minimum of one-year relevant clinical RN experience
• Strong computer and technology skills
