About the position

The Registered Nurse (RN) Case Manager at Enhabit Home Health & Hospice plays a crucial role in providing home-based patient care. This position involves coordinating care among various disciplines, ensuring effective communication with physicians, and overseeing patient visits. The RN Case Manager is responsible for maintaining accurate patient records and ensuring compliance with care protocols, all while contributing to a supportive and innovative workplace culture.

Responsibilities

• Perform visits and coordinate client care as the point of contact for all disciplines involved in patient care.
• Consult with physicians and the office regarding patient care details as needed.
• Perform accurate OASIS collection and ensure the medication profile is current.
• Ensure lab values are reported to the physician in a timely manner.
• Attend weekly case conferences and monthly case manager meetings.

Requirements

• Currently licensed as a Registered Nurse (RN) in the state of employment.
• A minimum of one year of clinical experience is preferred.
• Demonstrate knowledge and skill in current nursing practice.
• Possess a valid state driver's license and automobile liability insurance.
