About the position

InCare Ohio is seeking a dedicated and compassionate Registered Nurse Case Manager to join their healthcare team. This part-time role involves coordinating and managing the care of hospice patients, ensuring they receive high-quality healthcare services. The RN Case Manager will conduct patient assessments, develop individualized care plans, and collaborate with other healthcare professionals to provide comprehensive care.

Responsibilities

• Conduct patient assessments to determine their healthcare needs and develop individualized care plans
• Monitor and evaluate patient progress, making necessary adjustments to the care plan as needed
• Collaborate with physicians, specialists, and other healthcare professionals to ensure coordinated and effective care
• Educate patients and their families on their medical conditions, treatment options, and self-care techniques
• Administer medications and treatments as prescribed by physicians
• Document patient information accurately and maintain confidentiality in accordance with HIPAA regulations
• Provide emotional support to patients and their families throughout the healthcare journey
• Advocate for patients' rights and ensure they receive appropriate medical services
• Participate in case conferences and interdisciplinary team meetings to discuss patient progress and develop care strategies

Requirements

• Valid Registered Nurse (RN) license in the state of Ohio with no board action
• Minimum 1 year of experience in acute care or managed care setting
• Strong knowledge of medical terminology, physiology, and healthcare practices
• Proficient in conducting patient assessments and interpreting vital signs
• Familiarity with HIPAA regulations and ability to maintain patient confidentiality
• Excellent communication skills, both verbal and written
• Ability to work collaboratively in a multidisciplinary team environment
• Strong organizational skills with attention to detail
• Compassionate nature with a commitment to providing exceptional patient service
