Registered Nurse - RN Case Manager in Grass Valley, CA- Hybrid Remote

About the position

The RN Case Manager at DaVita Integrated Kidney Care is responsible for supporting complex patients with chronic kidney disease (CKD) and end-stage kidney disease (ESKD). This role involves navigating the healthcare system, assessing patient needs, and implementing care plans in collaboration with healthcare teams. The position is hybrid-remote, requiring both telehealth and in-person visits within a designated area, and operates on a flexible Monday to Friday schedule.

Responsibilities

• Support complex patients with CKD/ESKD by navigating the healthcare system.
• Conduct medical record reviews and consultations to identify patient needs.
• Implement appropriate interventions based on patient assessments.
• Work autonomously and collaboratively with healthcare team members.
• Coordinate care transitions and manage ongoing patient needs.
• Establish treatment plans in partnership with the care team.
• Identify and address social determinants of health affecting patients.
• Assess patients to identify unreported medical conditions or changes.

Requirements

• Current RN License is required; BSN preferred.
• Minimum of 2 years RN experience required with healthcare performance coaching.
• Current CPR or BLS certification is required.
• Current driver's license and willingness to travel within an assigned territory.
• Intermediate computer skills and proficiency in MS Word, Excel & Outlook required.
• Home office with internet connectivity at a minimum of 1MB upload and 1MB download speed required.

Nice-to-haves

• 2+ years of Case Management or Chronic Care Management experience.
• 1+ years of utilization management experience.
• Spanish-speaking preferred in some markets.
