About the position

The Registered Nurse (RN) Case Manager at St Croix Hospice is responsible for planning, organizing, and directing hospice care for patients and their families. This role emphasizes community health education and involves comprehensive assessments to determine hospice needs, developing care plans, and coordinating care with interdisciplinary teams. The RN Case Manager plays a crucial role in providing professional nursing care, ensuring patient comfort, and facilitating self-sufficiency in daily activities.

Responsibilities

  • Completes an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs.
  • Provides professional nursing care by utilizing all elements of nursing process.
  • Assesses and evaluates patient's status by writing and initiating plan of care.
  • Regularly re-evaluates patient and family/caregiver needs and participates in revising the plan of care as necessary.
  • Initiates the plan of care and makes necessary revisions as patient status and needs change.
  • Uses health assessment data to determine nursing diagnosis and develops a care plan that establishes goals.
  • Initiates appropriate preventive and rehabilitative nursing procedures and administers medications as prescribed.
  • Counsels the patient and family in meeting nursing and related needs and provides health care instructions as appropriate.
  • Assists the patient with activities of daily living and facilitates efforts toward self-sufficiency and comfort care.
  • Acts as RN Case Manager when assigned by Clinical Supervisor and coordinates patient care for assigned caseload.
  • Completes, maintains, and submits accurate clinical notes regarding patient's condition and care given.
  • Communicates with the physician regarding the patient's needs and reports changes in condition.
  • Teaches the patient and family/caregiver self-care techniques and provides medication and diet instructions.
  • Provides and maintains a safe environment for the patient and assists in continuity of care.
  • Participates in on-call duties as defined by the on-call policy and ensures necessary arrangements for equipment and services are available.
  • Supervises ancillary personnel and delegates responsibilities when required.
  • Actively participates in quality assessment performance improvement teams and activities.

Requirements

  • Graduate of an accredited school of nursing.
  • One (1) to two (2) years of recent acute care experience in an institutional setting.
  • One (1) to two (2) years of recent experience in home care preferred.
  • Current licensure in State.
  • Excellent observation, verbal and written communication skills, problem solving skills, and basic math skills.
  • Prolonged or considerable walking or standing; able to lift, position, and/or transfer patients.
  • Must be a licensed driver with an automobile that is insured and in good working order.

Nice-to-haves

  • Management experience not required but responsible for supervising hospice aides.
