About the position

The Registered Nurse (RN) Case Manager is responsible for planning, organizing, and directing hospice care, focusing on community health education and experience. This role involves assessing patient and family needs, developing care plans, and providing professional nursing care to ensure optimal comfort and support for patients in their homes and communities.

Responsibilities

  • Completes an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs.
  • Provides professional nursing care by utilizing all elements of nursing process.
  • Assesses and evaluates patient's status by writing and initiating plan of care and regularly re-evaluating patient and family/caregiver needs.
  • Participates in revising the plan of care as necessary and initiates appropriate preventive and rehabilitative nursing procedures.
  • Administers medications and treatments as prescribed by the physician in the physician's plan of care.
  • Counsels the patient and family in meeting nursing and related needs and provides health care instructions as appropriate.
  • Acts as RN Case Manager when assigned by Clinical Supervisor and coordinates patient care for assigned caseload.
  • Completes, maintains, and submits accurate and relevant clinical notes regarding patient's condition and care given.
  • Communicates with the physician regarding the patient's needs and reports changes in the patient's condition.
  • Teaches the patient and family/caregiver self-care techniques and provides medication, diet, and other instructions as ordered by the physician.
  • Participates in on-call duties as defined by the on-call policy and ensures that arrangements for equipment and other necessary items are available.
  • Supervises ancillary personnel and delegates responsibilities when required.

Requirements

  • Graduate of an accredited school of nursing.
  • One (1) to two (2) years of recent acute care experience in an institutional setting.
  • One (1) to two (2) years of recent experience in home care preferred.
  • Current licensure in State.
  • Excellent observation, verbal and written communication skills, problem-solving skills, and basic math skills.

Nice-to-haves

  • Management experience not required but responsible for supervising hospice aides.
