About the position

The Registered Nurse RN Case Manager at Lafayette Regional Health Center is responsible for ensuring high-quality, patient-centered rehabilitative care. This role involves managing and coordinating care specific to the needs of each rehab patient, collaborating with various healthcare professionals, and focusing on fiscally responsible strategies to deliver excellent care. The position allows nurses to spend more time at the bedside with patients, enhancing the overall patient experience.

Responsibilities

• Provide program orientation to patients, families, and caregivers regarding the case manager's role and rehabilitation philosophy.
• Act as the coordinator of patient/family/caregiver education.
• Promote participation of patients and families in team discussions related to care plans and goals.
• Ensure implementation of the patient's treatment plan that supports their strengths and preferences.
• Document findings of the Discharge Planning Evaluation and psychosocial assessments.
• Communicate the social, financial, or discharge needs of patients and families.
• Promote consistent, positive patient interactions to enhance patient service.

Requirements

• Valid RN License
• Associate Degree or Bachelor's Degree in Nursing
