The professional registered nurse Outpatient RN CAP 2 is an entry-level designation within the Clinical Advancement Program (CAP) for new employees who meet the RN CAP 2 criteria or have advanced from RN CAP 1. This role is crucial in providing and coordinating clinically competent care for an assigned group of patients, which includes assessment, planning, intervention, and evaluation. The RN CAP 2 is responsible for delegating appropriate tasks to direct patient care staff, including RNs, LVN/LPNs, and Patient Care Technicians. As a member of the End Stage Kidney Disease (ESKD) healthcare team, the RN participates in decision-making, teaching, leadership functions, and quality improvement activities aimed at enhancing patient care outcomes and facility operations. In this position, the RN performs essential functions under the direction of the Supervisor and with guidance from the Clinical Educator or Preceptor, collaborating with other Registered Nurses. The RN is tasked with the ongoing systematic collection and analysis of patient data before, during, and after hemodialysis treatment, documenting findings in the patient medical record, and making necessary adjustments to treatment plans. The RN also assesses and documents the learning needs of patients and their families to provide initial and ongoing education. Furthermore, the RN directs and collaborates with direct and ancillary patient care staff to ensure the safe and effective delivery of hemodialysis therapy. The RN administers medications as prescribed, initiates emergency response measures when necessary, and serves as a resource leader and mentor for new and existing employees. The RN ensures correct laboratory procedures are followed and participates in education and quality improvement projects as directed by the Supervisor. This role may also involve serving as a Preceptor to new employees and requires completion of CAP requirements to maintain or advance within the program.
