About The Position

The Registered Nurse (RN) in Home Health is responsible for planning, organizing, and directing home care services for patients in their homes and communities. This role emphasizes community health education and requires a commitment to providing quality care while embracing a diverse workforce. The RN will assess patient needs, develop care plans, administer treatments, and coordinate care with other health professionals.

Requirements

• Graduate of an accredited school of nursing.
• Current licensure in California and CPR certification.
• Minimum of 2 years licensed experience in nursing.
• Bachelor's degree preferred, with one year of home health care experience preferred.
• Excellent observation, verbal and written communication skills.
• Basic math skills and nursing skills per competency checklist.

Nice To Haves

• Management experience not required but preferred.
• Experience in discharge planning and home health care plans.

Responsibilities

• Completes an initial assessment of patient and family to determine home care needs.
• Provides a complete physical assessment and history of current and previous illnesses.
• Regularly re-evaluates patient nursing needs and initiates the plan of care.
• Develops a care plan based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
• Administers medications and treatments as prescribed by the physician.
• Provides direct patient care as defined in the State Nurse Practice Act.
• Counsels the patient and family in meeting nursing and related needs.
• Identifies discharge planning needs and implements them prior to patient discharge.
• Acts as Case Manager when assigned by Clinical Supervisor.
• Prepares clinical notes and updates the primary physician as necessary.
• Communicates with community health-related persons to coordinate the care plan.
