About the position

The Registered Nurse (RN) - Hospice Case Manager is responsible for coordinating care for terminally ill patients and their families in hospice settings. This role involves assessing patient needs, providing comfort care, educating families, and collaborating with an interdisciplinary team to ensure quality end-of-life care. The RN will manage a caseload, conduct regular patient visits, and maintain accurate documentation in compliance with healthcare regulations.

Responsibilities

  • Identify and coordinate patient/family care for terminally ill patients.
  • Conduct patient/family contacts at least every 15 days based on assessment of need.
  • Utilize teaching, assessment, and intervention skills to provide comfort care.
  • Educate patients and families on disease processes, management, and hospice philosophies.
  • Update primary physician as indicated and according to agency requirements.
  • Review all aspects of patient caseload for appropriate service utilization.
  • Make referrals to other team members as appropriate.
  • Administer medications and treatments as prescribed by the physician.
  • Lead the interdisciplinary care group (IDG) for each patient and family.
  • Develop, revise, and implement the patient's Plan of Care in collaboration with the IDG.
  • Coordinate services and collaborate with the interdisciplinary team for consistent patient care.
  • Arrange for necessary equipment and medical supplies.
  • Document patient experiences and assessments in the electronic health record.
  • Assess patient responses to treatments and adjust interventions as needed.
  • Communicate changes in patient condition to physicians and IDG members.
  • Supervise Home Health Aide and LPN visits according to agency standards.
  • Instruct paraprofessional staff in accordance with regulations.
  • Facilitate discharge planning and ensure continuity of care post-discharge.
  • Utilize proper body mechanics and safety techniques during patient handling.
  • Orient and mentor new staff as requested.

Requirements

  • Valid Registered Nurse license issued by the state of Illinois.
  • Basic Life Support (BLS) certification from the American Heart Association.
  • Completion of an accredited nursing program.
  • 1-2 years of nursing experience in home health, hospice, or related fields.
  • Competence in principles of deformity prevention and body mechanics.
  • Excellent communication and interpersonal skills.
  • Strong decision-making and critical thinking abilities.
  • Ability to work independently and prioritize tasks.
