About the position

The Registered Nurse (RN) Manager at Family Bridge Healthcare Services is responsible for conducting health assessments, monitoring individual care plans, and providing professional support to individuals in a home care setting. This part-time role involves managing care plans, implementing physician's orders, and ensuring compliance with healthcare regulations while maintaining a high level of professionalism and critical thinking.

Responsibilities

  • Conducts home visits at least every six months unless a change in individuals' health and environment condition.
  • Observe and monitor individuals' plan of care according to the physician's instructions.
  • Identify individual needs observed that require attention by supported staff and/or physician.
  • Assist and support management in specified tasks or assignments.
  • Administer medication and take vital signs according to physician's orders.
  • Inspect wounds and change dressings according to physician's orders.
  • Examine individuals for skin breakdowns and any signs of infection.
  • Communicate with family members or guardian and physician, as needed.
  • Educate Direct Support Professionals and family of the individuals on the aftercare or ongoing support and services for individuals.
  • Monitor individuals' recovery of health conditions when required, according to physician orders and compile reports for the physician.
  • Document home visit observations, vital signs and monitor the Service Record notes and charts completed by LPN's.
  • Provide resources and training for LPN's.
  • Review Medication Administration Record (MAR), Individual Service Plan (ISP) documents for tracking, individual's medication, and doctors' orders to ensure compliance at least monthly or whenever there is change in condition of the individual's health and/or environment.
  • Be attentive to individual's rights and responsibilities.
  • Comply with and follow HIPPA guidelines.
  • Perform other duties as assigned.

Requirements

  • Bachelor's Degree in Nursing from an accredited school of nursing preferred.
  • Must possess a current, valid RN license in state of practice.
  • 2 years of experience in a healthcare related field preferred.
  • Excellent communication skills.
  • Clear DBHDD or GCHEXS criminal background check.
  • Demonstrated ability to maintain a high level of professionalism.
  • Strong critical thinking skills.
  • Intermediate in computer software applications (Microsoft Office, Word, Excel, and Outlook).
  • Valid driver's license.
  • Familiar with Medicaid regulations, DBHDD programs and HRST certification preferred.
