Responsibilities

• Assess and monitor patient health status, including vital signs and medical history.
• Maintain accurate patient records and documentation.
• Collaborate with healthcare professionals to develop and implement individualized care plans.
• Complete 100 Initial Assessments.
• Complete 50 Supervisory visits.

Requirements

• Current Registered Nurse (RN) license in the state of practice.
• Experience working in a hospital environment is preferred.
• Strong communication skills to effectively interact with patients, families, and team members.
• Ability to work collaboratively within a multidisciplinary team.
• Commitment to maintaining patient confidentiality and adhering to ethical standards of care.
