About the position
The Registered Nurse Triage/Weekend Coordinator for Homecare at Middlesex Health is responsible for ensuring that home care patients receive all available services during extended hours, weekends, and holidays. This role involves processing new referrals and coordinating care in accordance with hospital objectives and standards. The position requires a commitment to providing high-quality care and support to patients in their homes.
Responsibilities
- Ensure home care patients receive all available services during extended hours, weekends, and holidays.
- Process new referrals from all referral sources.
- Coordinate care in accordance with Middlesex Hospital objectives, standards, and policies.
- Visit patients as required to provide care and support.
Requirements
- Registered Nurse with a degree from an accredited School of Nursing.
- Current RN license to practice in the State of Connecticut.
- Computer literacy and ability to work in multiple systems.
- Ability to multi-task effectively.
Nice-to-haves
- Homecare experience preferred.
- Certification in a specialty area preferred.
