Responsibilities

• Perform comprehensive patient assessments.
• Collaborate with a multi-disciplinary team including PT, OT, ST, MSW, and paraprofessionals.
• Supervise a caseload, nurturing the growth and development of LPNs and Home Health Aides.
• Facilitate care coordination and ensure seamless support for patients.
• Document patient journeys using OASIS assessments, SOC/Admissions, recertification, and physician orders.

Requirements

• Active RN License
• One (1+) Year of Clinical Experience
• CPR Certification
• Valid Driver's License
