About the position

HealthPRO Heritage is seeking a Full-Time Rehab Program Manager in Beaverton, OR, responsible for overseeing rehabilitation services in an Assisted Living Facility and two additional sites. The role requires a licensed Physical Therapist, Occupational Therapist, Occupational Therapist Assistant, or Physical Therapist Assistant with management experience. The Program Manager will maintain a clinical caseload, ensure compliance with therapy service delivery regulations, and lead a team of rehabilitation staff while promoting positive employee relations and community engagement.

Responsibilities

  • Maintains a clinical caseload.
  • Demonstrates strong clinical knowledge and understands the role of rehabilitation for all clinical disciplines and product lines within the Assisted Living, Independent Living and CCRC settings.
  • Ensures compliance with Federal, State and Local laws, rules and regulations related to therapy service delivery.
  • Assesses data reflective of the resident's status and provides care according to department policies and procedures.
  • Demonstrates good planning, organizational and interpersonal skills necessary to maintain a safe, orderly and goal directed work environment.
  • Acts as a liaison between HealthPRO-Heritage management, Home Health, Hospice and community administration.
  • Integrates clinical programs and addresses community-specific needs related to marketing.
  • Participates in the implementation of Quality Improvement Programs.
  • Provides classes and education to the community, physicians, and other healthcare providers regarding therapy and facility services.
  • Plans, schedules coordinates and directs rehabilitation services for the rehabilitation department.
  • Determines staffing needs and schedules to meet the demands of resident care.
  • Conducts performance appraisals of all rehabilitation personnel.
  • Monitors and assists with personnel compliance standards including required credentials and corrective actions.
  • Develops special programs for resident participation, such as fitness and dementia programming.
  • Supervises Rehabilitation Technicians, students and volunteers as needed.

Requirements

  • Oregon Licensed Physical Therapist, Occupational Therapist, Occupational Therapist Assistant, or Physical Therapist Assistant.
  • Previous management experience within a similar setting preferred.
  • Master's degree or Bachelor's degree in a relevant field.
  • Valid state professional licensure.

Nice-to-haves

  • Experience in Assisted Living or similar healthcare settings.
  • Knowledge of Medicare regulations and compliance standards.
