About the position

HealthPRO Heritage is seeking a part-time Rehabilitation Program Manager in McMinnville, OR, at Parkland Village ALF/ILF. The role requires an Oregon Licensed Occupational Therapist or Physical Therapist to manage rehabilitation services in a 22-bed Independent Living and 50-bed Assisted Living facility. The position involves maintaining a clinical caseload, ensuring compliance with therapy service delivery regulations, and providing leadership to clinical therapy staff while promoting positive employee relations and community engagement.

Responsibilities

  • Maintains a clinical caseload.
  • Demonstrates strong clinical knowledge and understands the role of rehabilitation for all clinical disciplines and product lines within the Assisted Living, Independent Living, and CCRC settings.
  • Ensures compliance with Federal, State, and Local laws, rules, and regulations related to therapy service delivery.
  • Assesses data reflective of the resident's status and provides care according to department policies and procedures.
  • Represents HealthPRO-Heritage professionally as the point of contact for community customer concerns or issues.
  • Acts as a liaison between HealthPRO-Heritage management, Home Health, Hospice, and community administration.
  • Integrates clinical programs and addresses community-specific needs related to marketing.
  • Participates in community-based committees to promote clinical programs and marketing goals.
  • Participates in the implementation of Quality Improvement Programs.
  • Facilitates open communication in patient care meetings while adhering to HIPAA guidelines.
  • Provides education to the community, physicians, and other healthcare providers regarding therapy services.
  • Plans, schedules, coordinates, and directs rehabilitation services for the department.
  • Determines staffing needs and schedules to meet resident care demands.
  • Conducts performance appraisals of rehabilitation personnel and collaborates on performance reviews.
  • Oversees documentation audits to ensure compliance with regulations and timely clinical documentation.
  • Monitors personnel compliance standards and assists in resolving staff and resident-related issues.
  • Develops special programs for resident participation, such as fitness and dementia programming.
  • Provides leadership to and directly manages clinical therapy staff.

Requirements

  • Oregon Licensed Occupational Therapist or Physical Therapist.
  • Master's degree or Bachelor's degree in a relevant field.
  • Associate's degree in an allied health field.
  • Typing abilities and basic computer knowledge.
  • Commitment to ongoing continuing education and professional development.

Nice-to-haves

  • Experience in Assisted Living, Independent Living, and CCRC settings.
  • Knowledge of Medicare regulations and documentation requirements.
