About the position
The Rehabilitation Specialist at Sheppard Pratt's Adult Residential Treatment Center provides essential support for individuals aged 18 and older with serious mental illnesses, including those with co-occurring substance use disorders or developmental disabilities. This role focuses on creating a safe and therapeutic environment that promotes client independence and well-being, while working collaboratively with a multi-disciplinary team to implement individualized service plans.
Responsibilities
- Act as a residential counselor, working one-on-one with clients to help them learn or regain skills for successful community living and socialization.
- Ensure medication compliance, including responsibility for safety, dosage, and cleanliness.
- Collaborate with a service team to develop and implement individual service plans for clients.
- Provide transportation for clients to medical appointments, offering advocacy services and ensuring follow-up instructions are documented and followed through.
- Develop rapport with clients, becoming part of their trusted support system and celebrating their progress in recovery.
Requirements
- High school diploma or GED; Bachelor's degree in a related field preferred.
- Willingness to obtain additional certifications and trainings as required.
- Valid driver's license with 3 points or less and access to an insured vehicle.
- Availability to work days, nights, weekends, and holidays as needed.
