About the position

The Release Services Case Manager at Catholic Charities of Los Angeles plays a vital role in the Time-Limited Subsidy Program, which is part of the LA Continuum of Care portfolio. This position focuses on providing case management and financial assistance to individuals experiencing homelessness, utilizing a Housing First approach. The case manager will work with participants to help them secure and stabilize in permanent housing, while also promoting self-reliance and stability through various support services.

Responsibilities

• Meet with participants who are unhoused once a week to discuss and update the housing search plan.
• Research relevant housing topics and organize information for participants and staff.
• Complete Homeless Management Information Systems (HMIS) training and manage client data effectively.
• Document participant status in the HMIS database.
• Conduct a minimum of one home visit each month after the participant has been housed.
• Analyze the need and frequency for home visits based on participant needs.
• Assess and reassess participants' needs, household budgets, and follow up on retention plans.

Requirements

• Master's degree preferred, Bachelor's degree required.
• Three to five years of related experience in case management.
• Experience working with the homeless population.
• Ability to work in a fast-paced, high-intensity environment.
• Skilled in comprehensive case management services, including assessment and service planning.
• Valid California Driver's License, Class C, and auto insurance.
