About the position

The Remote RN Care Manager is responsible for managing high-risk patients, supporting care transitions, conducting proactive outreach, and improving patient outcomes. This role is essential in addressing post-discharge needs to reduce hospital readmissions and enhance patient care engagement, ultimately delivering a superior patient experience.

Responsibilities

• Conduct proactive outreach to high-risk patients, focusing on preventive care and minimizing long-term risks.
• Maintain a caseload of 15+ patients, providing personalized care plans and timely follow-ups.
• Provide post-discharge support, including follow-up calls, to ensure recovery and address barriers to care.
• Partner with care teams and stakeholders to facilitate seamless transitions from hospital to home.
• Gather and record patient health histories, monitor conditions, and report findings to support care planning.
• Develop and implement targeted interventions to manage high-risk patients and reduce readmissions.

Requirements

• Active RN license
• Associates Degree or Higher
• A reliable high-speed internet connection (the faster the better!)
• Ability to pass a background check and drug test
• Private, quiet, and distraction-free workspace in a room with a closed-door
