REMOTE* Telehealth - PT for Nursing Home Therapy !

About the position

The Vetus Foundation is seeking a fully remote Physical Therapist (PT) to provide telehealth services for its nursing home therapy departments. This part-time position involves planning and administering skilled physical therapy treatments for patients with various injuries and functional disabilities, ensuring they achieve the highest level of physical function. The role requires collaboration with physicians and clinical teams to develop and implement effective rehabilitation programs.

Responsibilities

• Plan and administer prescribed skilled physical therapy treatment and training for patients.
• Test/screen the patient's physical strengths to assist in evaluating their level of function.
• Develop and implement a conditioning/rehabilitation program consistent with the physician's Plan of Treatment.
• Confer with physicians and clinical team members to obtain additional patient information.
• Provide written instructions/care plans to Physical Therapy Assistants and Home Health Aide staff.
• Accurately document patients' care observations, interventions, and evaluations.
• Report patient's progress to the physician, Clinical Manager, staff, patient, and family.

Requirements

• Degree from an accredited Physical Therapy Program (approved by the APTA).
• Minimum of one year physical therapy experience preferred.
• Current and unrestricted Physical Therapy licensure.
• Current CPR certification.
• Strong organizational and communication skills.

Nice-to-haves

• Home Health experience a plus.
• Compact Physical Therapy License preferred.
