About the position

The Resident Service Coordinator at NHE, Inc. plays a crucial role in supporting residents in Affordable/HUD subsidized housing, particularly those who are frail or at risk. This position focuses on empowering residents to live independently by providing referrals and services that meet their wellness and social needs. The Coordinator acts as a liaison between residents and community service providers, ensuring that residents receive appropriate services and support to age in place successfully. The role requires strong customer service and social service skills, as well as the ability to conduct assessments and coordinate services effectively.

Responsibilities

  • Conduct comprehensive, non-clinical assessments of residents for wellness and social needs.
  • Help residents identify, access, and coordinate services, including monitoring and follow-up communication with service providers.
  • Develop and arrange educational/preventative health programs and services for residents.
  • Maintain partnerships with community service providers and stakeholders.
  • Manage daily online time card entries and documentation of resident case management services.
  • Navigate local, state, and federal benefits and entitlements.
  • Adhere to HUD required CEU and training requirements.

Requirements

  • Bachelor's degree in a social service-related field or equivalent work experience as per HUD requirements.
  • Knowledge of elderly and disability services and referral processes.
  • Two to three years of experience in social service delivery, preferably with elderly populations or persons with disabilities.
  • Bilingual skills and training in cultural competency are preferred.
