About the position

The Service Coordinator at Les Chateau and Hopewell Senior Village is responsible for providing general case management and referral services to residents, ensuring they receive the necessary support to age in place. This role involves conducting assessments, developing individualized service plans, and coordinating with community service providers to meet the needs of older adults and individuals with disabilities.

Responsibilities

• Provide general case management and referral services to residents.
• Conduct intake assessments and evaluate health, psychological, and social needs.
• Develop individually tailored case plans for residents requiring formal case management.
• Routinely assess service needs and consult with residents returning from hospital or rehabilitation.
• Conduct annual health and wellness assessments for all participating residents.
• Establish links with community agencies and service providers to secure necessary services for residents.

Requirements

• Bachelor's Degree or at least two years of case management experience.
• Demonstrated working knowledge of community services available for older adults.
• Experience building relationships with local service providers and government agencies.
• Experience working with older adults in a direct service capacity.

Nice-to-haves

• Bachelor's Degree concentration in Social Work, Gerontology, Psychology, Sociology, Counseling, Public Health, or Therapeutic Recreation.
• Knowledge of eligibility requirements for federal and state entitlement programs.
• Ability to advocate, organize, problem-solve, and achieve results for older adults and people with disabilities.
